TO BECOME ELIGIBLE FOR WARRANTY COVERAGE, THIS PRODUCT REGISTRATION MUST BE COMPLETED, SIGNED BY BOTH DISTRIBUTOR AND DEALER AND PURCHASER, AND FORWARDED TO CHAMPION HOIST AND EQUIPMENT AT TIME OF DELIVERY. FAILURE TO COMPLETE THIS FORM, OR ANY FALSIFICATION ON THE FORM, WILL RESULT IN INVALIDATING THE WARRANTY. THE TERMS AND CONDITIONS OF THIS WARRANTY ARE APPLICABLE ONLY ON CHAMPION HOIST AND EQUIPMENT SOLD AND INSTALLED IN THE UNITED STATES, CANADA OR MEXICO. Fill in this Warranty Registration and mail within fifteen (15) days after purchase to Champion Hoist and Equipment – PO Box 947 – Dunn, NC 28334, or Warranty will be invalid.

We have made final inspection of the above listed equipment and find it to be free from any visual or functional defects. We have applied all required decals. We have included a "Body Prop" and "Prop Cup" to be used for safety with this equipment.
